What to Bring to the Appointment
Insurance and Payment Information Neurology & Pain Management, is a provider for Medicare and most major insurance plans. We provide insurance billing. Anything not covered by insurance will be your responsibility. We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan. If you have any questions regarding which insurance plans we accept or any patient billing concerns, please call us at the phone number below. Questions regarding your coverage and benefits should be directed to your employer or insurance company. If you need to check whether Dr. Ungar is in your network, please call Mikki at 219.866.7222. If you need to check the status of a past bill or insurance issues, please call Vadim at 847.679.2455. Please remember to bring insurance cards and I.D. to the office on your first visit or if you change jobs or insurance companies. Please inform us which insurance we should bill as the "primary" and which is "secondary" in case you have two insurances (for instance, Medicare and or Medicaid). Notice of Privacy Practices for Neurology & Pain Management Information for Out-of-Town Patients Referral service On receiving your medical file, we will review your case and advise as to an appropriate referral in your area. We have performed due diligence on most major cities in the US and UK. We are able to afford you our experience in many years of referral in neurology and pain management. Review of your records We are able to review your imaging studies, MRI's, CT scans and medical file in order to make an appropriate comment on your care to date and make recommendations for your future care. Out-of-Town Service We offer a two to four day overnight stay service in association with local motels and hotels (for which we receive a discount for our patients). We offer a pick up and return service to the airport, which is about an hour from all major airports in the Chicago land Metropolis. During this stay, we will examine you, perform appropriate neuro-diagnostic studies and imaging studies, where necessary, and pain management services in our affiliate surgi-center. Financial hardship for out-of-town patients Please do call our office regarding financial hardship so that we can better serve you with payment plans and other financial strategies that will alleviate the burden of your healthcare costs somewhat.
